ONLINE School Registration/Ensemble Tickets Form

  • To register, submit this registration form as soon as possible.
  • The festival will be limited to the first 8 ensembles that register and pay the registration fee.
  • If more than 8 bands register a waiting list will be posted on the website in the order the applications are received. If a band withdraws, or does not pay the registration fee by the posted deadline, the first band on the waiting list that has paid the registration fee will take their place.
  • If an ensemble on the waiting list has paid the registration fee and does not move onto the performance list by February 22nd 2008, the registration fee will be refunded.
  • The ensemble registration fee is $150.00 Checks or money orders only. (NO PURCHASE ORDERS).

  • Ensembles that withdraw after February 22nd, 2008 will forfeit the registration fee.

WE NEED THE FOLLOWING INFORMATION NOW SINCE TICKETS GO ON SALE SOON. WE NEED TO KNOW IF YOU ARE STAYING FOR THE EVENING CONCERT, AND HOW MANY TICKETS YOU WILL NEED AS WE CANNOT EXCEED THE CAPACITY OF THE AUDITORIUM.

   If so, how many tickets @ $10 each to see STEVE SMITH'S JAZZ LEGACY and THE WINTHROP UNIVERSITY JAZZ ENSEMBLE, will you be ordering for your students and chaperones to attend the evening performance? (Note: The actual ticket order and money will not be required until February 22nd, 2008. Slight changes in the number of tickets you order now are possible at that time. (Tickets for the general public are $20 each.)

Quantity of tickets your school will probably need. (You may bring additional non-performing students to the evening concert for $10 also. Include a Director's complimentary ticket in this total).

 

 

 Please estimate the driving time between your school and Northwestern High School
  (in hours).

 

A request for a full refund of the participation fee must be received in writing by schools that withdraw prior to February 22nd, 2008. Schools withdrawing after that date will forfeit the entire fee.

Note: In the event the festival is canceled after February 22nd, 2008 due to circumstances beyond our control, (i.e. weather) concert tickets will be refunded but not registration fees.
